SUPPORT COORDINATION
Our team will support you in understanding and implementing the funded supports as specified in your plan of care. Our team will facilitate and connect you to the community, stakeholders and other service providers. Starz Health will support you in building life skills and connecting you to services, negotiating with providers, and advocating for you regarding the costs of services. Our team will ensure service agreements and service bookings are completed. As part of Support Coordination, our team will provide, but will not be limited to, the following services:
- Support and assistance to ensure you can exercise choice and control, coordinate supports and access your local community.
- To assist in planning to prepare for your plan review.
- To assist and ensure that you are getting the most out of your funded supports.
SPECIALISED DISABILITY ACCOMMODATION
THERAPEUTIC SUPPORT
Some clients will require specialist housing solutions and specialised care solutions to ensure individualised care and that they receive optimal care in their extreme functional impairment or very high support/care needs. Starz Health will ensure and assist participants with very high support requirements in arranging Specialised Disability Accommodation. This appropriate accommodation will ensure that clients are provided with personalised care.

PLAN MANAGEMENT
Plan managing your NDIS funding with Starz Health ensures that you have access to support systems, offering you a sense of security and more autonomy over your day-to-day decisions. Starting and managing an NDIS plan can be stressful and confusing for many participants. Starz Health is run by a dedicated and friendly team, ensuring autonomy for our clients. We will provide personalised financial management and transparency, giving you peace of mind.
Our Plan Management support team will assist participants with care planning, but not limited to the following aspects:
- Pay your providers for the supports you purchase.
- Help you keep track of your funds.
- Take care of your financial reporting.
- Depending on your circumstances, we can also assist you in selecting your providers.
PARTICIPATE COMMUNITY
Starz Health will support and assist our clients with daily living and life skills. That is achieved by ensuring participation in the community, which leads to the development of participants’ social skills. We facilitate activities and ensure that clients attend community, social, and recreational events.
ASSISTIVE PROD- HOUSEHOLD TASK
Starz Health will ensure that you have access to the appropriate assistive products and equipment for which you are eligible under NDIS funding, as outlined in your care plan. Household tasks can be made easier with specialised aids and equipment for cooking, cleaning, washing, and home maintenance, ensuring you enjoy your life. That includes, but is not limited to:
- Alarms and doorbells.
- Adapted clocks.
- Programmable memory aids.
- Assistive devices for dishwashing and cleaning.
- Assistive devices for cooking.
- Environmental control units (ECUs).
HOUSEHOLD TASKS
Starz Health will support and assist clients in participating in various daily household tasks. Our support includes but is not limited to:
- Personal Care.
- Household cleaning
- Lawn and outdoor maintenance.
- Personal Skills Development.
- Dishwashing, clothes washing, linen and ironing, to maintain a safe, secure, and healthy environment for our clients.
COMMUNITY NURSING CARE
As part of our community Nursing Care, we provide various care services in the comfort of your own home. Our team will make arrangements with Hospital Discharge Planners, and we will follow up with clients in their homes to provide the necessary care.
Our Services include, but are not limited to:
- Specialised and general nursing.
- Wound care and management
- Continence management
- Palliative care.
- Medication administration and management.
- Peg Feeds.
- Tracheostomy care.
- Ventilated clients.
- Catheter insertion/Care.
- Working with the Allied Health Team to ensure care is delivered on time
- Post Discharge Care.
HOME MODIFICATION
Home modifications are changes to the structure, layout, or fittings of a participant’s home to ensure safety, access, and comfort. As per the NDIS, Home modifications may be included in your plan if:
As a result of your disability, you may not be reasonably able to access and use frequently used rooms and spaces in your primary residence, such as the toilet, standard rooms, etc.
Your primary residence, in its current condition, has significant and adverse impacts on the sustainability of current living and care arrangements; and
A suitably qualified Occupational Therapist has performed an assessment and recommended home modifications, considering all possible alternatives, including the use of equipment.
ASSISTIVE TRAVEL/ TRANSPORT
NDIS participants will generally be able to access funding for transport assistance if they cannot use public transport due to their disability. Starz Health assists participants to access assistive travel/Transport.
ASSIST- PERSONAL ACTIVITIES
Starz Health supports individuals to learn everyday living skills. We provide supported opportunities in a range of activities to achieve this. These activities help participants develop independence and social interaction skills. These activities include, but are not limited to:
- Cooking
- Cleaning
- Paying bills
- Budgeting
- Living independently
- Personal hygiene
ASSIST- LIFE STAGE, TRANSITION
Starz Health, as part of the NDIS Assistance, will ensure support coordination across life stages and transitions. That includes both short-term and long-term supports focused on strengthening a participant’s ability to coordinate their services and to assist them in living at home and participating in their community. That includes, but is not limited to:
- Support coordination
- Assistance with accommodation and tenancy obligations
- Life transition planning mentoring, peer support and personal development
- Support and assistance with daily planning, decision making, financial planning and budgeting
ASSIST- LIFE STAGE, TRANSITION
Through the NDIS, Starz Health supports participants in learning everyday living skills, resulting in personal development. We ensure this is achieved by implementing a variety of strategies, resulting in the development of independence and social interaction. These include but are not limited to:
- Cooking
- Cleaning
- Paying bills
- Budgeting
- Living independently
- Personal hygiene
ACCOMMODATION/ TENANCY
Starz Health, through the NDIS, provides participants with assistance and support services to help maintain their housing. We provide but are not limited to, the following support:
- Maintaining residences through support and training.
- Providing support to clients who live in their own home or reside in the family home.
- Assisting clients to find alternative accommodation at the same time, ensuring that participants develop the capacity to do this independently.
- By being an Advocate for a client, supporting negotiations with landlords, mediation, and housing conflicts/disputes on behalf of our clients.
- Assistance with fortnightly financial planning and budgeting.
- Providing and ensuring access to other support services.
